Travel Registered Nurse (RN) – Care Manager (Days)

Location : Vicksburg, MS

Start : ASAP

Duration : 13 weeks

Shift : Days, 8 : 00 AM – 4 : 30 PM with rotating weekend shifts

Requirements : Active RN license

BLS required

Minimum 3 years of acute hospital Case Management experience (travel experience preferred)

Role Summary :

The Travel RN Care Manager provides comprehensive case management services to ensure optimal patient outcomes, efficient utilization of resources, and effective discharge planning. Responsibilities include :

Coordinating and implementing individualized patient care plans

Collaborating with interdisciplinary teams to ensure continuity of care

Managing transitions of care and discharge processes

Utilizing EMR systems and referral platforms effectively
