    • What You’ll Do

  • Conduct initial in-person assessments for patients entering virtual care programs, developing care plans and coordinating goals with families.
  • Educate and empower patients and caregivers on program expectations, self-care, and treatment plans.
  • Identify and address social barriers to improve patient outcomes and reduce unnecessary readmissions.
  • Collaborate with interdisciplinary teams to ensure smooth discharges and strong continuity of care.
  • Seek opportunities to improve processes and outcomes, staying ahead of telehealth best practices .

    • Qualifications

  • RN license (Georgia or multi-state compact)
  • BLS required; ACLS preferred (within 180 days if not current)
  • Associate’s or Bachelor’s in Nursing (Master’s preferred)
  • 5+ years Home Health or Hospice experience preferred
