About the Role
The Life Skills Professional is responsible for client support through direct interaction throughout shift; collaboratively and actively assisting and supporting the participants of the program in the development of Life / Recovery Support / Coping / Daily living skills. Facilitate and document life skills groups, implementing the activities on the monthly calendar; promote and enhance the trauma-informed culture of the program and the establishment and attainment of goals identified in the resident's treatment plan.
Program Description
HEART Services is a licensed (Community Rehabilitation Residence) residential program that utilizes evidence based person-driven practices to provide services needed for individuals to maintain independence in the community. HEART Services has the program capacity to serve 14 residents who present with a mental health diagnosis and an acute medical condition. Residents served demonstrate moderate to severe functional impairments that make it difficult to adjust socially and personally. HEART Services partners with residents in their own apartments that are customized to their own liking, to learn the skills necessary to achieve success in an independent environment. HEART Services implements recovery oriented services that are focused on self-determination and incorporate empowerment, opportunity and choice.
