Job Duties :
- Coordinate nutritional care of residents and patients by completing nutritional assessments.
- Participate in care planning meetings as required.
- Provide nutritional counseling and education for residents and family.
- Completes comprehensive assessments on patients as required.
- Provides dietary in-service training as required.
- Under PDPM, identify, document, and treat nutrition related diseases for improved outcomes.
- Meet DOH / state / federal and company standards.
- Perform duties independently without training or direction.
- Perform other duties as assigned by management.
Required Skills / Abilities :
Good decision making and judgment capabilities.Ability to identify and solve complex problems.Great communication and interpersonal skills are a must.High comfort level with enteral nutrition, wounds, weight management, Hemodialysis.Comfortable working in a healthcare facility.Familiarity with state survey process and compliance with nutrition care regulations.Education and Experience :
Bachelor’s degree in nutrition, health, or related field.Must possess Registered Dietitian licensure within the state of operationsAt least one year of experience in a long-term care settingExperience with MDS 3.0.Experience with electronic charting such as Point Click Care is a plusPhysical Requirements :
Prolonged periods standing and walking.Must be able to lift up to 25 pounds at times.
