Nurse Practitioner Primary Care (PACE Program for Seniors)

Location : Stockton, CA Salary : $181,749 $254,499 + up to $25,445 in incentives

Join a mission-driven healthcare program dedicated to helping seniors live independently and with dignity. We are seeking a compassionate and experienced Nurse Practitioner to provide primary care in a PACE (Program of All-Inclusive Care for the Elderly) model. This role offers the opportunity to deliver holistic, person-centered care as part of a collaborative interdisciplinary team (IDT).

Key Responsibilities :

Perform in-person comprehensive assessments (initial, semi-annual, and as needed)

Develop and manage care plans in partnership with physicians and IDT

Support participant care in homes, hospitals, and long-term care settings

Participate in rotating night / weekend call coverage

Contribute to quality improvements by reviewing utilization data and outcomes

Requirements :

Graduate of an accredited School of Nursing

Nurse Practitioner license with prescriptive authority in California

At least 2 years of experience working with the senior population

Strong knowledge of chronic and geriatric care practices

Valid CA driver s license and proof of auto insurance
