External Client Care Coordinator

Service Area : Multiple communities across South Central Pennsylvania
At Comfort Keepers, we believe care coordination is more than logisticsits about elevating the human spirit. As an External Client Care Coordinator, youll be the face of our mission in the community, helping families navigate care decisions and ensuring services begin smoothly and confidently. Youll also play a key role in expanding our caregiving team by engaging with local networks and representing Comfort Keepers at community events.
What Youll Do
Guide families through the care journey respond to inquiries, schedule visits, and help them feel confident in their decision to begin servicesAssess client and caregiver needs to ensure safe, high-quality careCoordinate schedules and resolve issues , keeping communication flowing between caregivers, families, and our internal teamSupport caregivers and families with training, guidance, and compassionate problem-solvingMaintain compliance through accurate documentation and ongoing monitoringRepresent Comfort Keepers in the community engage with local organizations, attend events, and help identify potential caregiving talentParticipate in on-call rotations and management meetings to drive continuous improvementHandle administrative tasks while bringing fresh ideas to care coordinationWhat You Bring
