MedCadre is seeking a travel Clinical Pharmacist for a travel job in State College, Pennsylvania.

Location - State College, PA

Shift : Days & Evenings

Expected Hours - 40 hours a week (8 hours a day, 5 days a week)

Job Type : Contract (12 weeks +Extensions)

Job Overview :

A Clinical Pharmacist plays a vital role in ensuring the safe, effective, and evidence-based use of medications in inpatient settings. They collaborate with physicians, nurses, and other healthcare professionals to optimize drug therapy, improve clinical outcomes, and promote patient safety across all hospital departments.

Key Responsibilities

  • Review and verify physician orders for accuracy, appropriate dosage, route, and potential drug interactions.
  • Provide clinical recommendations for medication therapy based on patient condition, lab results, and current treatment guidelines.
  • Participate in patient care rounds to advise on pharmacotherapy decisions and dosing adjustments.
  • Monitor therapeutic drug levels and adjust medications as necessary.
  • Educate healthcare staff and patients on medication usage, side effects, and best practices.
  • Ensure compliance with hospital formulary policies and regulatory standards.
  • Support antimicrobial stewardship and other quality improvement initiatives.
  • Oversee preparation and dispensing of sterile and non-sterile compounded medications.
  • Maintain accurate documentation in the electronic medical record (EMR).
  • Contribute to the development of treatment protocols, order sets, and clinical pathways.

    • Required Qualifications :

  • 2 year experience as a Pharmacist in a hospital Setting
  • BLS (AHA)
  • PA Pharmacist License

    • Education :

  • Bachelor of Science in Pharmacy required.
  • A doctor of Pharmacy degree preferred.
