Summary :

Under the supervision of a Coordinator or Supervisor, serves as a master's level professional clinician performing a wide range of direct and indirect mental health and co-occurring substance abuse services within a specific program or area of assignment. Serves as a case manager, conducts therapy, and recommends referrals to various programs, program components and other services in the community. Consults with service providers. Responsible for extensive case documentation. Assesses challenging behaviors and emotional trauma by interviewing and observing consumers, reviewing appropriate records and developing a specific treatment plan often using a strength based model. Responsible for carrying out all activities of the program / sub-unit in such a manner that fulfills CMHA-CEI's mission, policies and procedures.

Position Responsibilities :

• Assists consumers in obtaining resources necessary for community living, including but not limited to housing, financial entitlements and medical care.
• Demonstrates knowledge of entitlement and assistance programs such as Social Security, SSI, Food Stamps, HUD Housing, Medicaid, etc. Utilizes this knowledge to assist consumers with eligibility requirements, application process, securing and maintaining eligibility.
• Performs intake evaluations / assessments which include diagnostic interviewing, assignment of DSM diagnosis, mental status examinations, substance abuse assessments, and obtaining personal, family, social, emotional, and medical information related to the presenting problem. May include preadmission and annual consumer review.
• Coordinates provided services to consumers on his / her caseload. Consults, cooperates, advocates and collaborates with consumers, family members, other Community Mental Health staff, and community agencies linking and mediating for the consumer's benefit.
• Maintains clinical documentation related to consumer treatment and intervention in accordance with Community Mental Health Authority requirements, accreditation standards and third party billing standards in a timely manner.
• Provides clinical intervention (individual, group therapy, family therapy and counseling, crises intervention, psychosocial rehabilitation and case management) to assigned consumers and their families.
• Interviews individuals and family members in crisis, as necessary. Assesses the type and nature of the emergency and triage as appropriate.
• Facilitates admissions to a psychiatric hospital in accordance with prescribed procedures for voluntary and involuntary hospitalization. Provides discharge planning in accordance with Community Mental Health Authority policies and procedures.
• Facilitates admissions and discharge planning in accordance with prescribed procedures for voluntary and involuntary hospitalization. Provides input on treatment planning for patient stay and aftercare.
• Individually or as a member of a team and in conjunction with the consumer, assists in the development of a comprehensive treatment plan individualized to meet the consumer's needs with measurable goals and objectives.
• May be required to lend professional expertise in peer training and small group in-servicing.
• Adheres to the expectations of participation in continuing education to maintain and increase professional competence which may include unit seminars or theoretical issues, in-service training programs for Community Mental Health Authority or other agencies, or conferences that are relevant to the clinical work.
• May administer and interpret intelligence, achievement, interest, personality and various psychological tests, if appropriately licensed
• As a team member conducts clinical case load review sessions obtaining feedback on diagnostic and treatment processes. May participate in interdisciplinary diagnostic / staffing and case review sessions to exchange information and identify appropriate level of care (intensity of services) and interventions.
• Provides assessment and ongoing monitoring of consumer's mental status, substance abuse, and physical status. Facilitates consumer's identification of specific and incremental behavior objectives toward goal attainment. Documents consumer's evaluation of progress on goals and objectives, making revisions as determined by consumer. Directly assists consumers in obtaining needed resources. Provides outreach case management and counseling services
• Participates in clinical supervision related to the effective treatment of consumers.
• Serves as a liaison to other agencies and attends interagency meetings as needed.
• May represent Community Mental Health Authority in meetings with community groups and agencies.
• May supervise, provide feedback, and support to academic interns, practicum students and / or volunteers.
• Attends and participates in staff, team and / or interdisciplinary team meetings as required.
• Maintains familiarity with Michigan Mental Health Code.
• Establishes and maintains appropriate working relationships with consumers, staff and outside agencies.
• Maintains productivity standards relative to the requirements of the position.
• Responsible for knowledge about consumer recipient rights and procedures governing them.
• Responsible for knowledge and adherence to all Community Mental Health Authority policies and procedures.
• Observes all rules of confidentiality as it relates to consumer information, both internally and in dealing with outside individuals and / or agencies.
• Maintains an acceptable level of attendance and work performance.
• Observes all rules of confidentiality, both internally specified by policy and procedure and externally required by law.
• Will perform all other essential job duties as assigned.
