American Traveler is hiring a Registered Nurse for a Hospice assignment focused on case management and skilled nursing care for home health hospice patients, requiring at least 2 years of acute care experience.
\n \n Responsibilities\n
Work in a home health hospice setting with a patient caseload of 12 to 15 familiesDay shift schedule, Monday through Friday, 5x8 hour shiftsPopulation includes patients and families admitted to hospice carePosition involves case management and comprehensive assessments for hospice admissions, skilled nurse visits, recertifications, and dischargesUse of Epic and Remote Client EMR platforms for documentationMay float to other units as neededResponsible for providing skilled nursing care and education to patients and caregivers according to the hospice plan of treatmentCoordinates and communicates closely with the interdisciplinary team to ensure smooth flow of care across the continuumResponsible for wound care management, symptom management, development and management of individualized care plans, and discharge planningAssists with family bereavement services following patient deathUses Epic and Remote Client for clinical documentationHoliday coverage may be required depending on unit needsTime off requests must be presented for approval prior to signing the contractLocal candidates within 60 miles are not accepted\n \n Requirements\n
Active RN license in Oregon requiredProof of application for Washington state RN license required at the time of considerationMinimum 2 years recent acute care nursing experience requiredPrior hospice experience required for first-time travelersFully vaccinated against COVID-19Ability to provide two professional references from the past two years, with at least one from a supervisory roleNo more than one DUI on recordMust have been separated from the previous health system for at least one year if returning as a traveler\n Benefits\n
