Primary Purpose of the Organizational Unit :

The Center for Youth, Family, and Community Partnerships engages in partnerships for healthy children and families by bridging research, policy, and practice. The Center’s mission is to build the capacity of families, service providers, researchers, teachers, and communities to ensure the health and well-being of children and youth.

Position Summary :

The Community Health Worker will work closely with the Project Manager and a care manager to screen eligible participants and provide clinical interventions. They will also track and record clinical sessions, attend weekly one-on-one and group supervision sessions, and coordinate with other team members throughout the intervention. THE CHW will also supply all case management needs for program participants. The Community health Worker will receive necessary training to complete program interventions.

Minimum Qualifications :

BACHELOR’S DEGREE; OR EQUIVALENT COMBINATION OF TRAINING AND EXPERIENCE. ALL DEGREES MUST BE RECEIVED FROM APPROPRIATELY ACCREDITED INSTITUTIONS.

Preferred Qualifications :

  • Exceptional organization skills and the ability to work both independently and as a part of a research team; a strong interest in working with diverse populations; excellent oral and written communication skills is preferred.
  • We are especially interested in applicants who are bilingual (English / Spanish).

Alternate Option :

    Key Responsibility :

    Program Tracking, follow up, and coordination with participants and team members

    Essential Tasks :

  • Record and track audio recordings of sessions. Plan and meet with other team members as necessary to coordinate referrals.
  • Keep documentation of program activities up to date and provide supervision team with report each week.

  • Recruit potential participants at community based organizations, events, and clinics.
  • Screen participants and coordinate with research assistants to provide baseline screenings.
  • Coordinate with local sites to secure weekly hours and presence at community events.

    Clinical interventions with program participants

    Essential Tasks :

  • Provide clinical interventions using the modalities of motivational interviewing, CBT, mindfulness, and conflict resolution.
  • Work one-on-one with program participants and provide resources as needed.
  • Call participants to provide reminder calls and follow up with participants as needed in cases of emergencies.
