• Resume and license verification attached
• References (3)
• Background checks including county, federal, multi-state, nationwide sex offender, HHS / OIG, GSA, Social Security Number trace, 2 Step PPD or Quantiferon or proof of positive PPD and chest X-Ray, SAM search, Iowa SING background

A resume and license verification must be attached for consideration.

New graduates may apply; experience in Skilled Nursing Facilities is preferred.
