Job Description
The Wound Care Nurse Practitioner reports to the Director of Nursing and supervises nurses and medical assistants.
Job Summary : Provides specialized onsite wound care in home, nursing home, and assisted living settings for patients with complex skin conditions. Performs assessments, develops care plans, educates patients / caregivers, and collaborates with healthcare teams to promote healing.
Qualifications : Master's or Doctoral degree in Nursing, valid NP license, wound care certification within 6 months, minimum 2 years of relevant experience, and ability to work independently.
Physical & Mental Requirements : Requires physical effort, good sensory function, adaptability, and emotional stability. Must have reliable transportation and a valid driver’s license.
Essential Functions : Reassess wounds, administer treatments, supervise staff, document patient interactions, coordinate care, follow safety protocols, and educate patients. Performs in-depth wound care procedures and maintains confidentiality per HIPAA.
Work Environment : Field-based, involving travel to various locations, exposure to biohazards, and adherence to safety standards.
