Accurate Home Care, LLC is growing in the area of Home Health. We are seeking a Physical Therapist Assistant to support patient centered care.
This position carries out effective treatment plans for patients.

Pay & Perks :
In additional to the cultural perks :
- $30-$40 per visit
- Mileage Reimbursement

Essential Job Duties and Responsibilities :
- Provide patient centered treatment visits in their individual homes, or within a Senior Living Community
- Implement client treatment plans with guidance from the physical therapist
- Instruct patients on physical therapy interventions, modalities and home exercise programs
- Deliver treatments / interventions for patients that allow for exceptional patient care
- Understand and comply with policy and procedures set forth by Accurate Home Care
- Maintain compliance with county, state and federal regulations affecting home health care
- Must be able to commute within the Twin Cities and surrounding Metro Area
- A valid Driver's License and reliable vehicle are required
- Additional tasks, as assigned by manager
Qualifications
Education Requirements :
Current Physical Therapist Assistant / PTA license in the State of Minnesota
Continuing Education Requirements :
Complete education annually, as required by governing bodies, and / or company policies and procedures.
Additional Requirements :
Ability to read, write and speak fluently in English.
In the last five (5) years, two (2) years of experience as a PTA that is relevant to the patient population assigned is preferred.
Working knowledge of Microsoft Office suite.
Excellent oral and written communication skills.
