Together these facilities offer patients the best rehab and nursing care they can obtain after their hospital discharge.

• Physical Therapist istant PTA is responsible to organize and conduct physical therapy programs planned and overseen by the Physical Therapist to facilitate rehabilitation of those residents who have experienced a decrease in functions.
• Physical Therapist istant PTA will ist in planning organizing developing and implementing Physical Therapy services under the supervision of the Physical Therapist to ensure that the highest degree of quality resident care can be maintained at all times.
• Physical Therapist istant PTA performs treatment programs for igned residents
• Physical Therapist istant PTA enters billing information in an accurate and timely manner
• Report resident progress in meetings i.e. UR Meeting Patient / Family Conference as deemed required or appropriate
• Consult with other members of the rehabilitation team to coordinate physical therapy with other theutic activities
• Provide residents with equipment as appropriate
• Chart notes in an informative and descriptive manner that reflects the therapy provided to the residents as well as the residents response to the therapy
• Perform routine doentation as required and in accordance with our elished charting and doentation policies and procedures.

Sign and date all entries made in residents medical record.

Cooperate with all departments and staff facilitating harmonious working conditions to promote optimal resident care

Requirements :

• Graduate of an accredited school of Physical Therapy istant PTA
• Licensed in NY State as a Physical Therapist istant PTA; license in good standing
• Previous sed nursing facility (SNF) experience highly preffered
• Work well under pressure & willing to work harmoniously with all staff
• Sincere desire to work with the elderly chronically ill and handicapped
• Excellent customer service ss
• Must be able to communicate well verbally with residents families and staff
• Treat residents and fellow staff with respect dignity and compion
• Covid19 vaccinations are encouraged but NOT MANDATED ; we continue to offer COVID19 vaccination based on eligibility guidance from CDC
