Essential Functions :
- Assist with dissemination of information to community members to recruit program participants.
- Conducts home visits and assessments. Provides education as appropriate.
- Tracks client information and provides proper follow up to care plan. Updates and coordinates the tracking of infant immunization records and medical appointments for mother and child.
- Serves as an advocate for the clients. Consistently demonstrates ability to link clients / families with community resources.
Acts as a liaison between programs that provide and serve clients to ensure smooth interaction.
- Maintain case load and see all consumers on a regular basis as determined by risk level and per supervisor instruction.
- Attend and assist in all required classes, meetings, in-service trainings, and community activities.
- Applies skills and knowledge in child abuse / neglect and domestic violence while interacting with clients.
- Demonstrates organizational skills in independently managing workload and related duties.
- Will develop positive trait service plans based on strengths within the participant and her family.
- Will maintain a minimum of home visits and telephone contact with all program participants based on level of case management (1,2,3)
- Will provide referral and follow up for any additional supportive services required by participant.
- Must maintain absolute confidentiality regarding clients.
- Document service coordination activities throughout the continuum of care.
- Assess service plan in consultation with the client, make appropriate changes to the service plan to ensure progress toward goals.
- Maintain ongoing contact with client and involved significant others to ensure adherence to the service plan.
- Provide reports and paperwork as required in a timely manner.
- Performs miscellaneous job-related duties as assigned.
Requirements :
- Healthcare Knowledge : Two (2) years in human services work
- Tech-Savvy : Proficiency in basic data entry computer skills.
- Strong Interpersonal Skills : Ability to communicate courteously and effectively with individuals from diverse backgrounds.
- Team Player : An industrious and cooperative nature to foster harmonious working relationships with clinic management, physicians, clinicians, and the public.
- Education : Bachelor’s degree in Human Services
Preferred Qualifications :
- Prior CPR / First Aid certification
- Knowledge of The Joint Commission regulations
- Bi-lingual in Spanish, Nepali, French, Arabic
