Pediatric Registered Nurse PRN

Under the direction and supervision of the Community PedsCare manager, this PRN RN is responsible for providing hospice and palliative care services to pediatric patients, and support, education, and advocacy to their families / caregivers.
This may include providing nursing care to palliative and hospice patients in their homes on the basis of the patient's individualized Plan of Care (POC) developed, routinely reviewed and revised by the Interdisciplinary Group (IDG).
The Primary RN uses critical thinking skills in making decisions regarding patient care that includes knowledge and integration of pediatric care, regulations / standards, resources, and data.
- Assesses and identifies the needs of the patient and family at initial contact and subsequent admission visit, collaboratively develops the plan of care within the scope of the Community PedsCare program with the patient, family and IDG.
- Provides ongoing case management, education, advocacy, and empowerment to pediatric patient, parent / guardian, to maintain safe care with dignity, and focus on quality of life.
Implements the individualized collaborative POC within program guidelines, with ongoing re-evaluation and revision.
- Performs care coordination with appropriate referrals and follow-up based upon the ongoing assessment of the patient and family / caregiver needs.
- Provides new patient referral follow-up contact, scheduling and completion of program admission / enrollments.
- Attends patient deaths for primary patients and when on call, if needed.
- Acts as a liaison between Community PedsCare and other agencies and professionals involved in the patients' care.
- Communicates with the program's pediatric physicains / APRN, attending physician, organizational and community providers to ensure pain & symptom management, care coordination and continuity of care.
- Participates in IDG meetings as a contributing member of the team in ongoing communication, coordination, and continuity of the patient and family care / service needs from the clinical / nursing perspective.
- Ensures timely completion of all discipline-specific and organizational documentation according to Community Hospice policies and procedures.
- Routinely participates in the provision of after-hours on-call coverage.
- Participates in sharing information with and / or maintains membership with other pediatric professionals in hospice / palliative care through participation in organizations such as the Hospice and Palliative Nurse Association.
- Consults the Community Hospice Ethics Committee, if needed.
- Participates in program, organizational, community in-service programs or presentations, as appropriate.
Required Licenses / Certifications :
- Current State of Florida nursing license.
- Current State of Florida driver's license with an acceptable driving record. Education / Experience :
- Any combination of education and experience that would provide required skill and knowledge for successful performance would be qualifying.
Typical qualifications would be equivalent to :
- Completion of a nursing program from an accredited school of nursing with a Bachelor's Degree in Nursing.
- Minimum of two (2) years pediatric nursing experience.
- Minimum of two (2) years nursing experience.
- Home health and / or hospital experience
Knowledge of :
- Principles and practices of nursing.
- Pediatric nursing.
Skilled at :
- Self-motivation, self-direction in work performance.
- Ability to solve problems independently.
