ResponsibilitiesThe role of the RN in this setting provides both direct and indirect patient care following the standards of professional performance.

The following are essential functions in this role : * Diagnose and treat the patient's response to actual or potential health problems in a busy physician practice outpatient environment.

  • An active member of the interdisciplinary rehabilitation team and works collaboratively with physicians, patients, care givers, other interdisciplinary team members, and outside agencies to assist the patient to their highest level of independence.
  • Ensures that the care is individualized and appropriate to the patient's needs and that the care is carried out comprehensively according to the plan.
  • This role requires the ability to analyze, synthesize, triage, and evaluate the patient's status, concurrent care needs, and incoming calls to ensure that the highest level of safe and effective patient outcomes are achieved utilizing Epic.

In this outpatient physician practice clinic, the RN is responsible for supporting the physician practice including assisting with procedures, medication renewals, prior authorizations, telephone triage, and patient education.

In addition, the nurse supports the Magee Lifetime Follow Up activities by providing consultation and guidance to the patients as needed and performing inpatient admissions from home.

ESSENTIAL DUTIES AND RESPONSIBILITIES* Integrates evidenced-based practice, clinical expertise, patients, their families / care givers preferences and cultural beliefs and values for delivery of optimal healthcare.

  • Utilizes evidence-based knowledge and advanced critical thinking and clinical skills necessary to provide and document care for people with physical disabilities and chronic illnesses during in-person clinic visits, while triaging all telephone encounters, and in electronic communication utilizing the Epic system.
  • Provides patient care and directs and supervises ancillary nursing personnel.* Coordinates educational activities and uses appropriate resources to implement individualized teaching with patients and their families as needed.
  • Contributes to always providing a safe and therapeutic environment and supports activities that promote the patient's functional goals and the prevention of complications.
  • Demonstrates effective oral and written communication skills and disseminates up-to-date inter-disciplinary communication regarding the patient's status.
  • Acts as a resource, clinical mentor, and role model for nursing staff and students and participates in activities such as hospital-wide committees.

OUTPATIENT CLINIC - SPECIFIC DUTIES AND RESPONSIBILITIES : * Supports all Magee Outpatient Programs including Lifetime Follow Up Care, SCI, Brain Injury, Concussion, Stroke, General Rehabilitation, Amputee, Pain, Complex Wound Care, Workers Comp, and Telehealth.

  • Assists the physicians with all activities including : smooth and efficient patient flow; communication to facilitate patient satisfaction;
  • patient preparation and rooming; obtaining accurate patient data including biometric data, medication lists, and active problem lists;

queues medication renewals; performs prior authorizations and assists with procedures.* Summarizes input on the patient's clinical status for the physician during or after treatment encounters and executes all physician orders and medical regimens.

Assists the physician with : performing procedures such as Botox and Baclofen pump administration; activities associated with the prescription drug monitoring program (PDMP);

and assisting with the ordering of necessary urine drug screens and labs.* Utilizes skilled patient telephone triage skills.

  • Assists the physicians by providing a safe and adequate synthesis of all patient-related phone calls and requests and triages patient care needs and responses accordingly.
  • Assembles appropriate patient data and documents all findings, interventions, and results appropriately in the electronic medical record.
  • Queues orders for prescriptions and outpatient therapies.* Responds in a timely manner to all patient messages and prescription line requests to insure safe and effective patient care.

Anticipates and resolves any actual or potential patient issues.* Performs all the necessary procedural steps required clinically and for insurance purposes to admit Medicare and non-Medicare patients from home for an inpatient rehabilitation admission as necessary.

  • Executes best practice initiatives to maintain and improve Patient Satisfaction.* Demonstrates flexibility, problem solving and conflict resolution skills.
  • Performs job duties outside of primary responsibilities when requested or assigned.* Maintains clinical competency and continues professional growth by participating in training and continuing education programs necessary for credentialing or re-credentialing within the identified time frames.
  • Expected to participate in or support quality, evidence-based practice and research of nursing governance initiatives.* Maintains professional and technical knowledge.
  • Maintains the privacy and confidentiality of all patient information in accordance with all legal and regulatory requirements.

Qualifications* Graduate of a Bachelor of Science degree in nursing (BSN) required.* Licensure as a professional registered nurse (RN) in Pennsylvania.

  • A minimum of 2 years of licensed professional nursing experience is required either in rehabilitation or a related area specifically related to caring for the disabled population.
  • Certified in BLS (Basic Life Support) required or to be obtained within 30 days of hire.Conditions of EmploymentCovid Vaccination is a requirement for employment at Jefferson for employees working at Jefferson's clinical entities or at the University.

If you are not currently vaccinated you will be required to receive the vaccination prior to hire date if you are offered employment, unless you request and receive an approved medical or religious exemption from Jefferson.

Conditions of EmploymentSince this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI finger print based federal criminal records check.
