As a Community Health Worker (CHW) Apprentice at St. John's Community Health (SJCH), you will play a crucial role in promoting community health and well-being by engaging with residents through various outreach initiatives.
This position involves building relationships within the community, providing health education, and supporting individuals in navigating the healthcare system.
You will work closely with healthcare providers at SJCH to address the unique needs of the community and enhance access to quality healthcare services.
As part of your role, you will participate in a comprehensive training program. This program focuses on holistic growth, skill-building, and the application of popular education and non-traditional methods to address health disparities in vulnerable communities.
Through hands-on experiences, critical discussions, and fieldwork, you will learn to analyze and create new solutions for community health challenges.
This apprenticeship program emphasizes personal empowerment, recognizing that an effective CHW is not only an expert in health education but also a leader in building relationships and engaging in compassionate community work.

Education, Experience and Knowledge
- High school diploma or equivalent (bachelor's degree in public health or related field preferred).
- Previous experience in community outreach or health education.
- Knowledge of Microsoft Office Suite, see computer skills below.
- Strong interpersonal and communication skills.
- Knowledge of community resources and healthcare services.
- Cultural competence and the ability to work effectively with diverse populations.
- Ability to work independently and collaboratively within a team.
- Bilingual English / Spanish (read, write, speak) preferred, not required, see language skills below.
Responsibilities
Performs a combination, but not necessarily all, of the following duties :
1. Community Outreach and Mobilization :
- Assist in conducting door-to-door outreach, phone banking, and participating in community events and health fairs to engage with community members.
- Support in leading and coordinating efforts to mobilize community members, encouraging their active participation in health-related initiatives.
- Help establish and maintain strong relationships with local community organizations, leaders, and stakeholders.
- Collaborate with community partners to create a unified approach to addressing health disparities.
2. Health Education and Empowerment :
- Assist in designing and facilitating health education classes and workshops based on community needs and healthcare provider referrals.
- Provide information on preventive care, chronic disease management, nutrition, and other relevant health topics.
- Empower residents to take control of their health by providing resources and support for self-advocacy and informed decision-making.
- Help create and distribute educational materials to support health promotion efforts.
3. Patient Advocacy and Care Coordination :
- Assist community members in understanding and accessing healthcare services.
- Provide guidance on available resources and support services.
- Collaborate with healthcare providers at SJCH to learn about coordinating care plans for individuals with chronic conditions or complex health needs.
- Facilitate communication between patients, providers, and other healthcare team members.
- Track and report on the progress of care plans and interventions.
4. Campaign and Event Coordination :
- Help organize and coordinate health campaigns, community events, and health fairs with the goal of raising awareness and engaging residents.
- Assist in designing and implementing strategies to ensure the success and sustainability of community-driven health initiatives.
5. Data Collection and Reporting :
- Maintain accurate records of community engagement activities, outreach efforts, and health education sessions.
- Compile data on community health trends, needs, and challenges.
- Assist in generating reports for internal evaluation and showcasing the impact of community-led initiatives.
6. Stakeholder Engagement and Advocacy :
- Support as a liaison between the community and the Chief Government Affairs and Community Relations Officer (CGACRO) and assist as needed.
- Help cultivate strong relationships with key stakeholders, including local healthcare providers, community organizations, and governmental agencies.
- Assist in fostering open communication to ensure a collaborative environment.
- Attend government, community, and private interest group meetings to stay informed about key community issues.
- Produce briefing memos before community meetings and detailed reports, summarizing discussions and key issues raised.
- Maintain awareness of community affairs and report to the CGACRO as needed.
- Help with coordinating office procedures to ensure efficiency in routine tasks and correspondence.
- Manage telephone inquiries, responding promptly and professionally.
7. Advocacy and Coalition Building :
- Assist in advocating for policy changes and resources that positively impact community health outcomes.
- Support in building and sustaining coalitions with like-minded organizations to amplify the community's voice in healthcare advocacy efforts.
8. Leadership Development :
- Help identify and nurture community leaders by providing training and support for effective health advocacy.
- Assist in organizing leadership forums and empowering individuals to become agents of positive change within the community.
9. Other Duties :
- Complete additional tasks as needed or as assigned by the Director of Advocacy & Community Engagement.
- Engage in continuous learning and development through the apprenticeship program, which focuses on personal growth, skill-building, and leadership in community health work.
