About The Position
Job Title : Physical Therapist - Home Health Job Specialty : PT Home Health Job Duration : 13 weeks Shift : Day, 5x8-Hour (8 : 00 AM - 5 : 00 PM) Guaranteed Hours : Not guaranteed Experience : Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience depending on specialty License : Must have license in hand Certifications : CPR, BLS Must-Have : - Proficiency in HomeCare HomeBase and Oasis- Reliable transportation, current driver’s license, and agency-required liability insurance Job Description : Provide, coordinate, and direct home nursing care according to physicians' orders and agency policies- Apply the nursing process competently- Responsible to the Clinical Manager- Supervise and evaluate Home Health Aides and Licensed Practical / Vocational Nurses- Perform home visits and attend periodic office meetings- Manage an average of 5-8 patients per day- Points system applies for different types of visits and activities, with a weekly productivity expectation of a minimum of 30 points- Conduct various types of cases, including Start of Care, Wound Care, and Follow-Up Visits- Willingness to travel up to 75 miles from the home clinic- Mileage reimbursed according to standard rates- Candidates must have home health experience and be willing to travel locally if necessary- No on-call requirement currently*Note : * Training will be provided if proficiency in necessary software is outside the last 6 months.
Flu vaccine requirement is unknown. Dress code is scrubs.
