About the Role :

The Medical Care Coordinator Quality Improvement plays a crucial role in ensuring that healthcare providers meet the Healthcare Effectiveness Data and Information Set (HEDIS) standards. The coordinator will work closely with healthcare teams to identify gaps in care and implement strategies to address these gaps effectively, by analyzing data and monitoring performance metrics. This role requires a professional with strong understanding of HEDIS and Quality Risk protocols. This is an in-person, full-time position that also includes front office duties Monday to Friday, 8AM to 5PM and one (1) Saturday per month, 9AM to 5PM.

Minimum Qualifications :

  • 2 years experience as a Medical Assistant
  • Experience in care coordination or case management within a healthcare setting.
  • Strong understanding of HEDIS measures and Quality improvement processes.
  • Proficiency with Microsoft Office, particularly advance skills in Excel
  • Medical Terminology

Preferred Qualifications :

  • Care Coordination or Case Management.
  • Experience with electronic health records (EHR) eClincicalWorks and data analysis tools
  • Knowledge of healthcare regulations and compliance standards.

  • Front Desk responsibilities with check-in and check-out patients
  • Insurance Verification
  • Focus and Results-Oriented
  • Bilingual English / Spanish a plus
  • Responsibilities :

  • Support compliance with HEDIS and other Quality Risk standards by collecting, validating, and reporting essential healthcare data.
  • Analyze patient data to identify gaps in care and develop action plans to address these gaps.
  • Collaborate with healthcare providers and teams to implement quality improvement initiatives.
  • Monitor and report on performance metrics related to HEDIS compliance
  • Perform chart reviews, and upload data to patients charts
  • Require heavy volume of outbound and inbound calls
  • Communicate with patients by phone, mail and EMR Patient Portal
  • Schedule Appointments as required
  • Work Front Desk scheduling, verify insurance, update demographics, and collect patient payments.
  • Require working 1 Saturday per month
  • Work closely with other healthcare professions, nurses, and other care team members.
  • Educate patients and families about preventive care and the importance of adhering to treatment plans.
  • Skills :

    The required skills for this role include strong analytical abilities to assess patient data and identify areas for improvement. Effective communication skills are essential for collaborating with healthcare teams and educating patients about their care. Use knowledge of HEDIS Organizational skills are necessary to manage multiple tasks and ensure timely follow-up on care initiatives. Overall, these skills contribute to the successful coordination of care and the achievement of HEDIS compliance goals.
