Job Summary : The Registered Nurse is responsible for providing safe and effective delivery of patient care through assessment, planning, implementation and evaluation of patient and family needs in the home care environment.
Directs and supervises the home health aide in providing quality patient care services. The RN is responsible for organizing and managing cases including coordination with other professional disciplines (PT-OT-ST-MSW).

QualificationsEducation Requirements and Other Requirements : Education Level : Graduate of an accredited school of nursing.
Certification / Licensure : Licensed in the state of Illinois. BLS certification required.Experience Requirements : Two years of experience in a health care setting is required.
Home health experience is preferred.Other Requirements : Driving and reliable transportation is a necessity.Working Conditions : Exposure Category 1 : Tasks that involve exposure to blood, body fluids, or tissues.
All procedures or other job-related tasks that involve an inherent potential for spills or splashes of them, are Category 1 tasks.
