About the Department

The Comer Mobile Unit Program provides primary care to children, schools & communities, primarily on the Southside of Chicago.

Each year, the Mobile Unit services about 1,000 school-aged and preschool aged children and adolescents, providing health exams, sports physicals, immunizations, health screening, health education, adolescent medicine and other services that are necessary for school entry or indicated because of the students’ medical needs.

The Mobile Unit Program also evaluates sick children and teenagers, assesses students for mental health needs, helps link children who do not have a doctor to a primary healthcare provider, and refers children in need of pediatric specialty care to the University of Chicago's Outpatient clinic or to subspecialists at other institutions.

Job Summary

The Community Health Program Coordinator will assist the Principal Investigator (PI) with the creation of a Mobile Unit Program dashboard by tracking Mobile Unit Program clinical activity, performing data abstraction, and data analysis, among other research tasks.

The incumbent will also assist with establishing and maintaining crucial relationships with community health centers, sub-specialty providers, and various community-based resources.

The successful candidate will work with members of the Comer Mobile Unit to increase patient access, streamline resources, and ensure the seamless delivery of healthcare and social services.

The Community Health Program Coordinator will also provide technical and general administrative support to the PI and program.

This at-will position is wholly or partially funded by contractual grant funding which is renewed under provisions set by the grantor of the contract.

Employment will be contingent upon the continued receipt of these grant funds and satisfactory job performance.

Responsibilities

Develop relationships with community health centers, sub-specialty providers , community-based resources and mental health resources in order to facilitate access for patients.

Familiarize staff and patients with resources such as southsidehealth.org, a comprehensive, web-based, wiki-like database for health and social service resources on Chicago’s South Side and enable patients and families to access resources including food pantries, housing support, mental health services, park district and after school programs, tutoring programs, job support, etc.

Abstract subject data from medical records / source documentation (EMR / EPIC record, paper record) and enter into Redcap databases that track referrals, adolescent assessments and additional data required for reporting to funders.

Assist with creation of Mobile Unit Program dashboard by tracking Mobile Unit Program clinical activity.

Conduct literature review if indicated.

Track all referrals and follow-up at regular intervals to ensure that patients receive the needed services.

Follow-up with families to address barriers to care, such as lack of transportation, missed work or school, etc.

Follow-up with sub-specialty offices to obtain consultation letters to place in patient’s medical record.

Facilitate enrollment in All Kids, the state of Illinois’s medical coverage program.

Manage patient incentive program including dispersing incentives to families (gift cards, etc.).

Maintain and order equipment and supplies.

Work with the PI and Comer Mobile Unit team members to monitor the overall progress of the project.

In collaboration with PI, plan and implement systems and processes to optimize results.

Helps monitor compliances with institutional, state, and federal regulatory policies, procedures, directives and mandates.

Tracks vaccine inventory and maintains VFC vaccine supply if needed, including monitoring vaccine storage temperatures.

Prepares and submits vaccine inventory reports.

Scans select paper documents and manages upload of medical documents into EPIC.

Track upload of documents into EPIC to ensure that visit information in EPIC is complete.

Provides technical and administrative support for a research project.

Collects and enters data. Assists in analyzing data. Assists with preparation of reports, manuscripts and other documents.

Performs other related work as needed.

Minimum Qualifications

Education :

Minimum requirements include vocational training, apprenticeships or the equivalent experience in related field (not typically required to have a four-year degree).

Work Experience :

Minimum requirements include knowledge and skills developed through 2-5 years of work experience in a related job discipline.

Certifications :

Preferred Qualifications

Education :

Bachelor’s degree in a field relevant to the medical field or data management.

Experience :

Experience with data analytics.

One year of hands-on involvement supporting community program research.
