About The Position
Job Title : Physical Therapy Assistant Job Specialty : Rehabilitation Therapy Job Duration : 13 weeks Shift : Monday to Friday Guaranteed Hours : 32 hours per week Experience : New graduates are welcome;
Skilled Nursing Facility experience preferred License : State License Verification required Certifications : Not specified Must-Have : - Resume- Three professional references- License verification- Prescreen filled out and attached- Covid test 72 hours prior to start date with results issued 48 hours prior Job Description : Candidates must ensure all prescreening paperwork is completed and attached before submission.
- Include a resume and license verification in your application.- Complete a background check, including : - County Criminal (7 years for all counties lived and worked) - Federal Criminal (7 years or more) - Multi-State Criminal Search (7 years or more) - Nationwide Sex Offender Search (7 years or more) - HHS / OIG Search - GSA Search - Social Security Number Trace- Complete a 2 Step PPD or Quantiferon test or provide proof of Positive PPD and Chest X-Ray.
- SAM Search required.- Candidates will undergo an Oregon State Background check (facility run).- Ensure results for the Covid test are ready 48 hours prior to the start date.
