Responsibilities
- Provide physical therapy services including assessment, evaluation, procedures, and teaching activities as outlined in the patient Plan of Care
- Update care plans on an ongoing basis and submit completed visit notes as requested
- Perform all OASIS time point assessments and discharge planning process per Medicare Criteria
- Attend meetings, conferences, and educational in-services as required
- Ensure the use of the 4Ms and provide Age-Friendly Care
Qualifications
- Current, unencumbered State Physical Therapy License
- One year experience as a home care professional Physical Therapist preferred
- Proficient in clinical skills and Microsoft Office
- Ability to drive 2-4 hours per day
Compensation
USD $85,500.09 - $99,247.66 per year
