JOB SUMMARY
The Registered Nurse (RN) Case Manager advocates for patients, coordinates care, and facilitates efficient healthcare transitions.
This role involves collaborating with patients, families, caregivers, and community agencies to develop and implement care plans.
The RN Case Manager also ensures compliance with regulatory requirements and participates in performance improvement activities.
EDUCATION, EXPERIENCE AND QUALIFICATIONS
- Associate Degree in Nursing required; BSN preferred
- Minimum 1 year of discharge planning, case management, or RN experience in a medical setting
- Florida RN license and BLS certification required; Case Management Certification preferred
