Registered Nurse (RN) - Case Management

Duration : 13 Weeks
Shift : Days (M-F 8 AM - 4 : 30 PM, 7.5-hour shifts with a 1-hour lunch)
Role Summary :
This role is for an experienced RN in the Case Management. You will be responsible for discharge planning and collaborating with an interdisciplinary team on Med-Surg or Telemetry units.
You'll work closely with social workers, physicians, and other team members to ensure smooth transitions of care for patients, including referrals to skilled nursing facilities (SNFs), community resources, or home settings.
Unit Details :
- Expected to round on the unit with physicians and the team to plan discharges.
- The role is primarily focused on discharge planning, not utilization review.
Responsibilities :
- Develop discharge plans and referrals for patients.
- Collaborate with interdisciplinary teams, including social workers and physicians.
- Manage care transitions for patients with varying needs such as homelessness, wound care, and hemodialysis.
- Be knowledgeable of housing arrangements and community support services.
- Utilize EMR (Allscripts) for documentation and coordination.
- Occasionally, weekend and holiday shifts may be required with manager approval.
Requirements :
- BSN required.
- Minimum 1 year of inpatient case management experience.
- Prior critical care experience is preferred.
- Certifications : BLS, PRI (Patient Review Instrument), SCREEN Training Certification (can be taken online).
- Must have the ability to function independently with strong time management and critical thinking skills.
- Experience with CarePort, MCG, and InterQual is preferred.
- Familiarity with discharge planning for a diverse patient population is a must.

- Business casual attire or any color scrubs (excluding royal blue or maroon) is required.
- Department-specific orientation provided, with 1-2 weeks of preceptor support.
- Supportive management and a strong focus on interdisciplinary collaboration.
