Registered Nurse (RN) - Long Term Care
Job Overview :
We are seeking a compassionate and experienced Long-Term Care Registered Nurse (RN) to join our dedicated healthcare team.
The ideal candidate will provide high-quality care to residents in a long-term care facility, ensuring their physical, emotional, and psychological well-being.
The LTC RN will play a critical role in overseeing daily patient care, monitoring health conditions, administering medications, and collaborating with other healthcare professionals to ensure the best possible outcomes for our residents.
Key Responsibilities :
- Deliver skilled nursing care to residents, including administering medications, treatments, and wound care as prescribed.
- Assess, plan, implement, and evaluate patient care in collaboration with other healthcare team members.
- Monitor resident health status and report any changes to physicians and care teams.
- Maintain accurate and up-to-date medical records, including care plans and nursing notes.
- Educate residents and their families about their health conditions, treatment plans, and medication management.
- Support daily living activities such as mobility assistance, personal hygiene, and nutrition.
- Ensure compliance with all facility policies and state and federal regulations.
- Participate in care planning meetings to develop individualized care plans for residents.
- Foster a positive, supportive, and compassionate environment for residents and their families.
- Respond promptly to residents' needs and concerns to enhance their quality of life.
- Supervise and mentor Certified Nursing Assistants (CNAs) and other support staff.
Qualifications :
- Current and valid Registered Nurse (RN) license in Vermont or compact license.
- Minimum of 1 year of nursing experience, with experience in long-term care or geriatrics preferred.
- Strong clinical skills in patient assessment, medication administration, and care coordination.
- Excellent communication and interpersonal skills to interact effectively with residents, families, and healthcare teams.
- Ability to manage multiple priorities and work in a fast-paced environment.
- Compassionate and empathetic with a commitment to providing high-quality patient care.
- Familiarity with electronic medical records (EMR) systems preferred.
- BLS / CPR certification required.
Shift :
5x8 Hr. Nights (40 Hrs. per Week) (10 : 45pm - 7 : 15am)
