Job Description
- Transition inpatients to the clinic by seeing them within one week of hospital discharge and continuing the plan of care developed on the inpatient service (including follow-up of lab tests and procedures).
- Provide medical care to walk-in patients.
- Provide new patient medical evaluations and ordering of initial laboratory tests.
- Order pertinent diagnostic tests and procedures.
- Provide scheduled medical care to attending physician's patients unable to see their PCP (due to vacations, hospitalist month, etc.).
- Provide cross-coverage for other PA's as needed.
- Provide medication refills to patients throughout the clinic as needed.
- Complete appropriate documentation of all evaluations, care, and services rendered in the medical record.
- Make appropriate referral for consultations.
- Communicate with referring or primary care physicians regarding patient care and treatment.
- Provide follow-up and health maintenance care including the appropriate adjustment of medications in accordance with established protocols or in response to specific instructions from the Attending Physician.
- Consults with colleagues, supervising physicians, specialists, and other health professionals as appropriate.
- Attend morning clinical conferences.
- Provide medical care for patients in the chronic disease collaborative.
- Provide medical care for patients in the Coumadin Clinic.
- Provide medical care for pre-operative patients needing medical clearance.
- Review results of all labs / studies ordered
- Participate in Quality / Performance Improvement projects.
- Attend bi-monthly administrative meeting.
- Complete all related duties as assigned.
Minimum Qualifications
1. Current New York State registration as a Physician Assistant; or
2. Current limited permit issued by New York State to practice as a Physician Assistant.
