Registered Nurse (RN)

The Health Educator will develop and oversee the education and maintenance of consumers' health plan in a residential setting.

This position will partner with a multidisciplinary team to optimize the healthcare needs of individuals served who suffer from developmental disabilities, Mental Illness, and traumatic brain injuries.

The Health Educator is responsible for meeting the health care needs of consumers in the rehabilitative / habilitative setting, across the lifespan, and in the face of chronic and acute illness / injury.

Registered Nurse Roles and Responsibilities :

• Ensure resident compliance with general expectations of a comprehensive health care model including medication management, appointments, resident weights / vitals, medical record management / accuracy.
• Monitor medication efficacy and side effects.
• Completion of Plans of Care and annual Health Care Appraisals (HCA).
• Maintain compliance with Monthly / Quarterly reporting requirements.
• Coordinate attendance to all external appointments (PCP, Psych, Specialty)
• Participate in on-call rotations
• Educate / Train staff and residents

RN Qualifications and Education Requirements :

• Registered Nurse (RN) with current Michigan license
• High level of Nursing ethics
• Working knowledge of commonly used medications and treatments
• High level of responsiveness with appropriate use of nursing interventions

RN Required Skills :

• Proven excellence in time management, organizational and communication skills.
• Proficiency in Microsoft Office applications (Outlook, Word, etc.)
• Utilization Management
