New Program Job Opportunities for : Housing Medical Case Manager, Supervisor of Housing Case Management, Housing Counselor Bebashi-Transition to Hope is excited to announce the launch of a new transitional housing and supportive services program for people with HIV in Philadelphia.

We are seeking dedicated professionals to join our team and make a difference in the lives of our clients. Explore the opportunities below.

1. Housing Medical Case Manager Responsibilities :

  • Provide case management services to approximately 20 clients with HIV.
  • Complete Comprehensive Assessment and Plan (CAP) with clients.
  • Assist clients in locating permanent, affordable housing and achieving viral load suppression.
  • Maintain regular contact with clients and update their files.
  • Conduct home visits and accompany clients to medical appointments.

Qualifications :

  • Bachelor’s degree in Social Work, Human Services, Sociology, Psychology, or Nursing (Master’s preferred).
  • Expertise in HIV case management and client advocacy.
  • Knowledge of Philadelphia housing resources.
  • Flexibility to work evenings and weekends.
  • Fluency in Spanish is a plus.

2. Supervisor of Housing Case Management Responsibilities :

  • Supervise 2 Housing Medical Case Managers (MCMs) and 2 Housing Counselors.
  • Monitor caseloads and ensure project measures are met.
  • Conduct client home and field visits and provide crisis intervention.
  • Oversee client information collection and ensure confidentiality.
  • Generate monthly reports and participate in staff meetings and training.

Qualifications :

  • Master’s degree in Social Work.
  • At least 4 years of supervisory and program management experience in HIV services.
  • Knowledge of Housing First principles and Philadelphia housing resources.
  • Flexibility to work evenings and weekends.

3. Housing Counselor Responsibilities :

  • Identify and secure affordable rental housing units for clients.
  • Conduct housing inspections and establish relationships with landlords.
  • Provide rental subsidies and support for moving expenses.
  • Advocate for clients and assist in resolving housing issues.
  • Collaborate with Housing MCMs to secure permanent housing for clients.

Qualifications :

  • Bachelor’s degree in Human Services or related field, OR 3+ years of housing counseling experience.
  • Knowledge of Housing First, and trauma-informed care.
  • Experience with Coordinated Entry platforms for housing applications.
  • Ability to work with diverse populations and handle flexible hours.
