As part of our team, you will be traveling to patients' homes throughout the day, and all documentation will be done through an electronic device (smartphone) provided by Lorian Health .

We are looking for a full-time Licensed Vocational Nurse (LVN) who will provide care to our patients in different areas of Riverside and San Bernardino County.

As an LVN, you will provide direct nursing care to clients, as prescribed by the physician, under the direction and supervision of the Registered Nurse (RN).

Nursing activities are limited to those in compliance with the State LVN Practice Act, any applicable licensure / certification requirements, and Lorian Health policies and procedures.

We are a Pay Per Visit compensation model. Pay Per Visit rates range from $30 / visit - $40 / visit commensurate with experience and depending on type of visit.

In this role, you will travel to patients' homes and provide nursing care as prescribed by the physician. You will observe the patient / client regularly for changes in condition, discuss changes with the patient / client or family / caregiver, document and report changes as necessary to the attending physician.

You will also educate and instruct the patient, patient's family, and other caregivers regarding the patient's care.

Qualifications for the position include :

  • LVN / LPN-licensed to practice in the state of practice and in good standing.
  • Graduate of an approved school of practical / vocational nursing.
  • One (1) year experience as a professional nurse in an acute setting.
  • Possesses and maintains current CPR certification (BLS required).
  • Possess and maintains a valid driver’s license.
  • Possess and maintains an automobile that is insured in accordance with LORIAN requirements.
  • Ability to read and follow written instructions and document care given in the patients’ electronic medical record.
  • Possesses excellent observation, good nursing judgment, and good oral and written communication skills.
  • Is self-directing and has the ability to work with little direct supervision.
  • Ability to work with adults (18 years and older), demonstrating tact and patience.
  • Is flexible and cooperative in fulfilling the role obligations.
  • Participates in in-service education programs and clinical meetings.
  • Protects patients and employees by adhering to infection-control policies and protocols.
  • Maintains confidential patient records in accordance with privacy and security standards of the Health Insurance Portability and Accountability Act (HIPAA) and other applicable state laws.
