Ageless Living Home Health is seeking a dynamic ADON / RN Case Manager to join our growing home health agency. We are looking for a career minded individual who will represent our mission statement and meet the standards and expectations we uphold.
This is an office based position located in Colorado Springs, CO.

The Position :
We are looking for a full-time Assistant Director of Nursing / RN Case Manager with experience in Private Duty Home Health, RN Case Management, or related position.
Job duties include but are not limited to :
- Effectively oversees the management of patient care and the associated clinical staff (RN, LPN, CNA, etc.) in the field
- Evaluates appropriateness of each patient’s plan of care and revise on an ongoing basis with communication to all other disciplines
- Perform comprehensive assessments of physical, emotional, psychosocial and environmental needs of all assigned patients
- Effectively works alongside the DON / Chief Clinical Officer to ensure the continuity, quality and safety of services delivered in compliance with state and federal regulations.
Position Requirements :
- Must have extensive knowledge of state and federal guidelines and familiar with the home health industry
- Must possess the ability to lead and motivate agency staff
- Must work well under pressure, be detail oriented, have excellent communication skills and be a team player with proven decision making skills
- Must possess strong clinical skills.
- Must be a Registered Nurse and be of good standing with the Board of Nurse Examiners in the State of Colorado
- Pediatric Home Healthcare is a plus

Pay : $80,000.00 - $95,000.00 per year
