Job Summary for Registered Nurse Case Manager (RNCM)
- Plan, organize and direct hospice care services, from initial evaluation and assessment, pain and medication management, and care plan development.
- Ensure high quality patient experience and positive health outcomes through a comprehensive and personalized plan of care, proactively making revisions based on patient needs.
- Prepare complete and timely clinical notes, coordinate care delivery with an interdisciplinary team and collaborate with the patient's primary care physician to ensure continuity of care.
- Provide coaching and education to patients and their caregivers based on clinical diagnosis, empowering them to self-manage their condition, prevent complications, and improve quality of life as a Registered Nurse Case Manager (RNCM)
Minimum Qualifications
- Current CA Registered Nurse (RN) license
- Current CPR card for Healthcare Providers
- 1 year of Registered Nursing experience required
- 1 year of Registered Nurse (RN) in hospice preferred
- Must be a licensed driver with an automobile that is insured in accordance with state / or organization requirements and is in good working order.
- Demonstrates excellent observation, problem-solving, verbal and written communication skills; nursing skills per competency checklist.
- Self-directed and able to work with minimal supervision.
