Community Health Worker (CHW) - Mission Services (Per Diem)

It includes a 473-bed community teaching hospital, a 76-bed inpatient psychiatric facility in Westport, a large multispecialty provider group, and special needs services for adults and children.

St. Vincent’s is the first hospital in Fairfield County to be integrated into the Hartford HealthCare network, and is the system’s second-largest hospital.

Project Overview : For this project, incumbent will be responsible for work related to two grants. There will be a focus on Breast Health, encouraging community members to receive screening mammography and promoting breast health.

In addition, incumbent will focus on identifying health-related social needs and work to meet those needs through available resources and community connections.

The incumbent will serve as part of the Mission Services / Community Health team and will be involved in many community health related programs.

The community health worker will :

• Connect patients / community members and families to resources and services that address their social influencers of health.
• Provide referrals and connection to service.
• Participate in bi-weekly food distribution.
• Attend health fairs and other community-based health promotion events.
• Participate in Medical Mission program.
• Assist with CHW caseload as needed to ensure continuity of care.
• Ongoing data entry tracking client progress and interactions. If your resume / does not include / your CHW experience which may be paid or unpaid please make sure to either add the experience to your resume OR include a cover letter that outlines your experience. Education :
• High School diploma or equivalent. Licensure :
• In process of working towards the CT Community Health Worker certification required.
• CT CHW Certification preferred Experience :
