Registered Nurse Case Manager

- Responsibilities :
- Provide home-based care to patients, oversee care team, and report patient progress
- Educate patients and family members on care plans, medication, therapy, and home safety
- Coordinate patient discharge from services
Requirements :
- Active, unrestricted RN license in the State of Michigan (BSN not required)
- At least one year of clinical experience as a Registered Nurse
- Current CPR Certification
- Reliable transportation with valid state driver's license and car insurance
