Per Diem Physical Therapist, Home Health
Join our compassionate team and play a vital role in helping patients achieve optimal physical function and independence.
- Plan and administer skilled physical therapy treatment for patients with injuries, illnesses, and disabilities
- Conduct physical strength tests to evaluate patients' function and develop treatment programs
- Collaborate with physicians and clinical team to create and adjust therapy treatment programs
- Provide supervision and support to Physical Therapy Assistants and Home Health Aide staff
- Accurately document patient care and progress reports
Required Experience / Skills :
- Degree from an accredited Physical Therapy Program
- Minimum one year of physical therapy experience preferred
- Current and unrestricted Physical Therapy license and CPR certification
- Strong organizational and communication skills
- Valid driver's license, auto insurance, and reliable transportation
