HealthCare Support is actively seeking a Nurse Practitioner / Physician Assistant to fill an opening with a large Healthcare provider based in the Peoria, AZ area.

Daily Responsibilities for Nurse Practitioner / Physician Assistant :
- This provider will specialize in Nephrology and work with patients remotely and in the field to assess and form appropriate treatment plans
- Communicates with the designated care team to maintain plan of care and share changes in patient conditions
- Mon-Fri : 8am 5pm
- Remote and Field-based role
Required Qualifications for Nurse Practitioner / Physician Assistant :
- Licensed Nurse Practitioner OR Physician Assistant
- At least 2 Years of experience as an NP / PA
- Must be able to meet background check and drug screening requirements
- Comfortable communicating with patients and care teams in-person and telephonically
Preferred Qualifications for Nurse Practitioner / Physician Assistant :
Experience in Geriatrics, Primary Care, or Nephrology

Pay Details : $115,000 - $150, 000 per year (based on experience)
