ABOUT THE JOB
Deschutes County is currently accepting applications for a Public Health Educator II - Community Alignment Specialist & Perinatal Care Coordinator for the Family Connects program within the Family Support Services team, which is part of Clinical & Family Services in Deschutes County Public Health.
Family Connects (FC) is an international, evidence-based, nurse home-visiting program, universally offered to families with newborns in Oregon.
The Community Alignment Specialist (CAS) will work within the Family Connects team to develop and implement an outreach plan to raise awareness about Family Connects for both healthcare providers and the community.
This position will be responsible for fostering established relationships with community partners, ensuring good communication regarding FC, identifying gaps, and finding solutions to address community-identified needs.
In collaboration with other team members, this position will plan and coordinate quarterly advisory board meetings as well as plan and lead smaller meetings with community partners.
This position is also part of the Perinatal Care Coordination (PCC) team. The PCC model is designed to increase access to medical care and enrollment into available perinatal resources during the prenatal and postpartum period for individuals and their families.
This is an onsite Monday through Friday, position with occasional evenings or weekends to attend program outreach as needed.
Key Responsibilities :
Maintain and strengthen relationships, partnerships, and coalitions with community resources that provide support to families and that participate in the community system of care for young children and their families.
Identify and maintain an updated list of resources for families and identify gaps in needed community resources so that the broader community can be induced to grow these resources over time.
Attend nurse case conference meeting to provide individual-case consultation about community resources to support nurse home visitors as they connect families with community resources, as needed.
Market the program by communicating with referral sources (for example, prenatal practitioners, hospitals, pediatricians, and outreach events), communicating with groups that interact with families, and soliciting support from community leaders.
Work within the FC and PCC teams to ensure community involvement in FC though an advisory board and other means of including feedback (such as focus groups or round tables).
Compile and maintain records, reports, and documentation of program activities regarding community relations for use in program evaluation.
Conducting PCC visits with pregnant or newly post-partum individuals, connecting them with resources, providing OHP enrollment assistance, and WIC certifications.
Provides service navigation support and care coordination.

Compensation
$5,851.99 to $7,842.22 per month for a 172.67 hour work month. Excellent County benefit package when eligible. This union-represented is available immediately.
BENEFITS :
Our robust health plan is offered at a minimal cost of $95 per month for employee only or $116 per month for employee plus dependents, which includes an innovative on-site clinic, pharmacy, and wellness and wellbeing services.
