JOB SUMMARY

An OT provides professional services within the scope of occupational therapy practice standards in collaboration with primary care physicians and other members of the health care team and teaches and educates patients and their families.

OT in the home is performed under the supervision of the RN case manager, Administrator, DON. If Nursing has opened the case, Occupational Therapy is a

secondary discipline and therefore works collaboratively and professionally with the RN case manager to provide necessary services.

QUALIFICATIONS

An individual who :

Graduated after successful completion of an OT education Program accredited by the Accreditation Council for OT Education (ACOTE) of the American OT Association (AOTA) or successor organizations and is eligible to take or has successfully completed the entry level certification exam for OTs from the NBCOT.

A currently licensed in MI.

Will have a minimum of two (2) year experience as a OT, preferably in a homecare setting. Criminal check & Current CPR certification.

If educated outside of the United States must meet both of the following :

  • Graduated after successful completion of an OT education program accredited as substantially equivalent to OT entry level education in the US by one of the following : Accreditation Council for OT Education (ACOTE), successor organizations of ACOTE, the World Federation of OTs, a credentialing body approved by the American OT Association, successfully completed the entry level certification exam for OTs developed & administered by the National Board for Certification in OCC Therapy (NBCOT).
  • On or before December 31, 2009, is licensed or otherwise regulated, if applicable, as an OT by the state in which practicing.

RESPONSIBILITIES / ESSENTIAL FUNCTIONS

Receives intake referral information and prepares OT paperwork / tools necessary for the visit. Continuing OASIS datat sets.

Obtains pertinent medical history from patient, family or significant others.

Performs home safety check and environmental assessment of the patient’s home environment. Performs OT evaluation or skilled visit and documents accordingly.

Performs physical examination and review of all body systems and documents such accordingly. Develops an appropriate and effective Plan of Care (POC) to be submitted to the physician for approval and implementation.

Evaluates the patient’s ADL and iADL abilities. Assists the physician in evaluating level of function.

Functionally reassesses the patient for on-going therapy services.

Helps develop the plan of care (revising if necessary) and at least every 30 days.

Develops and implements the HHA plan of care when HHA services are ordered. Revises and signs this care plan the beginning of each certification period if nursing has already discharged out of the case.

Supervises the HHA in accordance with state / federal requirements and documents the supervision if OT is the only discipline on the case & HHA is ordered.

Establishes OT goals based on the patient’s current level of functioning and potential for improvement.

Reviews billing processes with patient and / or family advising patient and / or family when co-pay or Medicare is not likely to pay for services.

Effectively communicates with patient and family the POC and progression of such. Keeps the patient informed ongoing.

Effectively instructs patients and families in the prescribed OT therapeutic, self-care and other activities that are directed at improving independence and physical and / or mental functioning.

Effectively communicates with other disciplines on the case.

Communication with the patient’s physician (verbally and / or in writing) to obtain OT treatment modalities and / or assistive therapy modalities to effect the best means to obtain the desired outcome for the specific medical problem that caused the case to open and / or recertify.

Instructs patients and / or families in the safe use of OT assistive devices as per orders. Communicates in the case conferencing sessions.

Submits accurate, complete paperwork at the end of every week.

Submits requests for re-authorization of more visits needed prior to third party insurers authorized number of visits expiring.

Participates in the Performance Improvement Committee process as requested.

Performs clinical record reviews (CRR) per agency policy in collaboration with the Nursing Supervisor (Peer Review). Submits the CRR timely.

Participates in staff meetings and in-services ongoing.

Participates in the growth of the office by being a willing preceptor for newly hired, same discipline employees.

Participates in the planning, operation and evaluation of the therapy services of the organization. Maintains professional licensure and actively seeks out educational experiences to enhance the practice of home care occupational therapy for her / him and the benefit of the patients serviced.

Notifies the Agency of emergencies, sickness, and other imminent occurrences that may affect the patient caseload as quickly as possible relative to the event’s occurrence.

Submits written time requests 2 weeks or more in advance of planned time off. Other duties as assigned.

JOB SUMMARY

A Physical Therapist provides professional services within the scope of physical therapy practice

standards in collaboration with primary care physicians and other members of the health care team and teaches and educates patients and their families.

QUALIFICATIONS

Successful completion & graduate of a PT Program approved by the Commission on Accreditation in PT education (CAPTE) or successor organizations of the CAPTE or an education program outside of the United States determined to be substantially equivalent to physical therapist entry level education in the US by a credentials eval organization approved by the American PT Association or an organization identified in 8 CFR 212.

15(e) as it relates to physical therapists.

A current, active license to practice Physical Therapy in MI.

A minimum of two (2) years’ experience as a Physical Therapist. Criminal background check & Current CPR certification.

If trained outside of the United States before January 1, 2008, meets the following requirements :

  • Was graduated since 1928 from a PT curriculum approved in the country in which the curriculum was located and in which there is a member organization of the World Conference for Physical Therapy.
  • Meets the requirements for membership in a membership organization of the World Confederation for Physical Therapy.

RESPONSIBILITIES / ESSENTIAL FUNCTIONS

Performs clinical record reviews (CRR) in collaboration with the Nursing Supervisor (Peer Review). Submits the CRR timely.

families. Professional skilled nursing is performed under the supervision of the Administrator / Director of Nursing.

QUALIFICATIONS

Graduate of an approved professional nursing program (RN) Currently, licensed in good standing in MI

Minimum of one (1) year of experience as a professional nurse, preferably in home care. Must have a criminal background check.

Must have current CPR certification.

RESPONSIBILITIES / ESSENTIAL FUNCTIONS

Furnishes those services requiring substantial and specialized nursing skill. Initiates appropriate preventative and rehab nursing procedures.

Prepares & submits timely, clinical and progress notes. Oversees coordination of patient services.

Informs MD and other staff of changes in PT situation or needs. Completes medical history / home safety check / environmental assessments.

Performs socio-psychological evaluation.

Performs assessment visit and documents timely. (ie. OASIS / Skilled Nursing Note etc.). Performs physical examination and review of all body systems and documents accordingly.

Develops individualized Plan of Care (POC) to be submitted to the physician for approval and implementation and performs necessary revisions based upon patient needs.

Determines medical necessity for other services. Regularly re-evaluates the patient’s nursing needs.

Develops and implements the HHA plan of care when HHA services are ordered. Revises and signs this care plan the beginning of each certification period.

Supervises HHA in accordance with state / federal requirements and documents the supervision. Orders other professional services that may be appropriate to the needs.

JOB SUMMARY

A Speech Pathologist / Therapist provides professional services within the scope of Speech Pathologist practice standards in collaboration with primary care physicians, teaches and educates patients and their families, and evaluates and assesses patient’s therapy needs.

QUALIFICATIONS

A person who has a masters or doctoral degree in speech-language pathology and who meets either of the following requirements :

  • Licensed as a speech-language pathologist in the state in which the individual furnishes services.
  • In the case that the individual provides services in a state that does not license speech-language pathologists, has successfully completed 350 clock hours of supervised clinical practicum (or in the process of accumulating supervised clinical experience) AND performed not less than 9 months of supervised full-time speech-language pathology services after obtaining a masters or doctoral degree in speech-language pathology or a related field AND has successfully completed a national examination in speech-language pathology approved by the secretary.

Must have a criminal background check. Must have current CPR certification

RESPONSIBILITIES / ESSENTIAL FUNCTIONS

Records type of treatments and patient’s response on clinical progress notes which are written on the day of the visit and incorporated into the patient’s medical record within 7 days.

Assists the physician in evaluating the level of function.

Helps develop an appropriate and effective ST Plan of Care (POC) to be submitted to the physician for approval and implementation, revising as necessary and at least every 30 days.

Functionally reassess the patient for on-going therapy. Determines medical necessity for other services for the case.

Selects and administers diagnostic and therapeutic ST techniques and materials. Effectively communicates with patient and family the POC and progression of such.

Effectively communicates with other disciplines in the case (case conferencing).

Communicates effectively with the Director of Nursing / Nursing Supervisor scheduled visits planned and changes to the schedule on a weekly basis.

Instructs and counsels other health team personnel and family members in methods of assisting patient in improving, correcting and accepting his / her speech disabilities.

RESPONSIBILITIES / ESSENTIAL FUNCTIONS

Provides OTA services under the supervision of the OT. Follows an established POC for each patient.

With the OT, assists in the eval and implementation of the rehab program for the patient. Performs ongoing safety check and environmental assessment during scheduled visits.

Effectively communicates with patient and family. Participates in educating the patient and family.

Effectively communicates with their supervisor to problem solve as situations arise.

Communicates in the case conferencing sessions, as appropriate, to establish best practices for the individual patient’s needs.

Participates in the QA Committee process as appropriate. Participates in the monthly staff meetings and in-service programs.

JOB SUMMARY

A registered nurse who provides professional psychiatric nursing services within the scope of nursing practice standards in collaboration with primary care physicians, teaches and educates patients and their families.

RN has documented experience in caring for psychiatric patients.

QUALIFICATIONS

Graduate of an approved professional nursing program (RN). Currently, licensed in good standing in MI.

Minimum of one (1) year of experience as a professional psychiatric nurse in an inpatient setting. Must have a criminal background check and current CPR certification.

Meets federal / state requirements for psychiatric nursing practice as well as company’s psychiatric nursing standards.

Possess skills necessary to work with psychiatric populations : child / adolescent psychiatric patients and / or substance abuse patients.

Accurate knowledge of psychotropic medications and their side-effects.

Possess knowledge related to cultural / spiritual needs of the patient population they serve.

RESPONSIBILITIES / ESSENTIAL FUNCTIONS

Perform psychiatric nursing assessment of patients referred.

Provide appropriate referrals based on ongoing assessment of mental / psyche status. Maintain a productivity standard established by the company.

Familiar with case management and reporting to the appropriate designated staff. Submit all documentation 24 hours following their scheduled home care visit.

Responsible to know when recertification is due on all assigned patients.

Responsible to leave required documentation in patient’s home, and maintain travel folders for visit notes that are kept in a locked box in the trunk of their vehicles.

Responsible for giving a weekly update of all assigned patients to their assigned supervisor. Attend all scheduled staff meetings / case conferences and be prepared to present their patients for interdisciplinary review when requested.

Participate in periodic reviews of patient’s medical records.

Able to accurately provide their patients with assessments, evaluations, and treatment plan development / review / updates.

Able to coordinate care with interdisciplinary staff and community liaisons. Maintain confidentiality of all patients and patient’s medical records.

Conduct themselves in a professional manner within the standards of the agency as well as within the standards set by the American Nurse’s Association.

Act as role models in the community and dress appropriately. Responsible to maintain their licensing / certifications.

Pass the agency medical competency examinations with a score of 80% Possess knowledge to plan / implement in crisis situations.

Maintain appropriate therapeutic boundaries.

Psychiatric Nurses will be competent in utilizing appropriate communication skills. Make appropriate community resources for assigned patients

Exercise good judgment and provide appropriate therapeutic interventions. Maintain company’s high standards of nursing practice at all times.

Psychiatric Nurses will provide in-service education related to psychiatric nursing to staff, as requested. Maintains professional licensure and actively seeks out educational experiences to enhance the practice of home care nursing for her / himself and the benefit of the patients serviced.

Notifies Agency of emergencies, sickness, and other imminent occurrences that may affect the patient caseload as quickly as possible relative to the event’s occurrence.

Submits written time requests 2 weeks or more in advance of planned time off. Other duties as assigned.

JOB SUMMARY

Under the supervision of the Physical Therapist and according to established treatment plans and specific procedures, provides treatment to physical therapy patients in order to assist them in regaining physical mobility and function, increase strength, and decrease pain.

QUALIFICATIONS

A person who is licensed, registered or certified as a PTA, if applicable, by the

state in which practicing, unless licensure does not apply, and meets the following requirements :

  • Graduated from a physical therapist assistant curriculum approved by the Commission on Accreditation in Physical Therapy Education of the American Physical Therapy Association.
  • Current PTA license in the state of practice.
  • A minimum of two (2) years experience as a PTA. preferably in home health care.
  • Criminal background check & and other state required checks for compliance.
  • Current CPR certification.

If educated outside the United States or trained in the United States military, graduated from an education program determined to be substantially equivalent to PTA entry level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association or identified at 8 CFR 212.15(e); and

  • Passed a national examination for physical therapist assistants.
  • Has a current PTA license in the state of practice.
  • Has a minimum of two (2) years experience as a PTA, preferably in the homecare setting.

RESPONSIBILITIES / ESSENTIAL FUNCTIONS

Assists PT in the implementation of individual patient physical therapy care plans; performs daily physical therapy assignments to accomplish patient therapy plans and treatment goals.

Responsible for the daily care of assigned patients per patient POC.

Guides patients through prescribed exercise programs, including passive, assistive, and resistive exercise routines designed to develop or enhance strength, muscle tone, and range of motion.

May participate in the development of home exercise programs designed to meet individualized patient needs.

Applies heat, light, water, and electrically based treatments / modalities (i.e., ultrasound, infra-red lamp, muscle stimulation, traction, hot and cold packs, paraffin baths, whirlpool, diathermy, etc.

to stimulate muscle / nerve function.

Assists patients in gait training (i.e., the use of parallel bars, walkers, crutches, canes, braces, etc.) in order to improve ambulatory skills.

Performs other therapeutic activities such as massage and body mechanics instruction.

Observes patient condition during treatment and notes any significant change in patient condition. Seeks assistance from PT or medical personnel when appropriate.

Records patient response to treatment and overall progress in patient charts. Refers patients to PT for reevaluation as necessary.

Prepares patient referrals as requested by PT.

Advises professional team members of patient progress and perceived effectiveness of treatments; may suggest revision of treatment plan to better meet patient needs and capabilities.

Escorts / transports patients to physical therapy area, giving verbal guidance to maximal physical assistance as required;

obtains wheelchairs, carts, walkers, etc. and assists patients into chairs and onto carts and table as necessary and provides physical support at all times.

Prepares physical therapy equipment.

Cleans and maintains therapy equipment by cleaning and changing mats, changing linen, cleaning and refilling equipment, etc.

Follows sterile procedures and guidelines and ensures that all equipment is working properly and reports needed repairs to supervisor.

Assists Physical Therapist in coordinating patient load. Participated in educating the patient and family.

Prepares charges for services provided and enter charges into computer if required.

Remains abreast of developments in the field of physical therapy technology by pursuing a program of self-development, attending in service and continuing education programs, etc.

Participates in in-service programs. Participates in QA program.

Performs related duties as required.
