A healthcare organization in New York City is currently seeking a licensed Registered Nurse (RN) to join their team as their new Coordinator of Care.
In this role, the Coordinator of Care (RN) will assume the ongoing, primary responsibility to coordinate, implement and continually evaluate the home care needs of patients.

Responsibilities :
The Coordinator of Care (RN) will :
- Review patient insurance information
- Coordinate activities involved in each individual care plan
- Make sure that pertinent findings are shared with family and professional caregivers
- Perform other duties, as needed
Qualifications :
- 1+ year of experience in a Medical-Surgical and / or Hospital setting
- Bachelor's Degree in Nursing
- NYS Registered Nurse (RN)
Desired Skills :
- Master's Degree in Nursing
- Bilingual a plus
- Home Care background
- Previous CHHA experience
