Location : Huron Valley PACE Ypsilanti, Mi
Huron Valley PACE serves Washtenaw, Monroe, Western Wayne, and parts of Oakland and Livingston counties providing participants a wide range of medical and supportive services in our Day Clinic located in Ypsilanti, Michigan or in the comfort of their own homes.
The Huron Valley PACE team of healthcare professionals specialize in working with the senior community to help participants and their family members make positive healthcare decisions and receive the care they deserve.
What you'll do :
Performs patient care activities, including maintaining optimal physical and emotional comforts toward achieving maximum self-reliance and independence within the Day Clinic or participants homes.
Activities will include providing basic ADLs, documentation, reporting changes in participants conditions, and other tasks as assigned to assist the primary care, therapy, and healthcare staff.
Works under the direct and indirect supervision of the Participant Care Manager but is independent in the application of nursing knowledge and skills.

What it Takes :
- Must be currently certified as a Certified Nurse Aide, and have proof of eligibility to work as a CNA in the State of Michigan
- CPR certification is required
- Must have reliable transportation and a valid MI driver’s license
- Minimum of 1 year of experience working with the elderly is required
