Additional Details
- AHA or ARC Only for BLS certification
- Proof of vaccine and 1 booster or exemptions must be submitted for approval
- Proof of 2 immunizations, positive IGG titer, if negative titer, must have booster or Declination accepted for MMR
- Proof of 3 immunizations and positive antibody titer, if negative titer, booster or Declination accepted for Hepatitis B
- Required from October - March (unless otherwise indicated by CDPH) Medical / Religious exemptions will be sent for approval with a physician's statement(medical) or written statement from traveler(religious) for Influenza Vaccine
- Proof of vaccination within past 10 years or Declination accepted for Tdap
- Required on start date for clinical staff for Covid Test
- HLC Testing to be completed by Friday before Start. If completed and confirmation provided Friday prior to start, a $300 stipend will be paid
