Job Details
- No candidates with an AZ home address on their drivers license will be considered - please include a copy in submission

- The form needs to be 100% accurate and have all requests noted in the highlights section.
- Start Date should be on or very quickly after the Order Start Date listed.
- All RTO should be listed on the cover page, must be less than 7 days.
- RTO will not be approved in the first 2 weeks of assignment.
- All facilities the traveler is open to working in should be listed on the submission.
- If interested in 'any' location, please include what states (I.E. Any AZ, Any CO, Any state, etc.)
- All shifts the traveler is open to working on should be listed on the submission.
- All departments the traveler is open to working must be checked on the cover page.
- All candidates submitted should be preclosed and ready to accept.
Job Requirements
- Minimum 24 months of current Level I Trauma ICU experience - REQUIRED
- MICU experience required.
- Must have previous continuous ICU experience for the last 4 years - REQUIRED
- May float to the other ICU units as needed, NSICU, CVICU, and STICU - REQUIRED
- Travel experience - REQUIRED
Schedule Information
- RTO will not be approved in the first 2 weeks or holidays.
- Can not exceed 7 days.
Unit Specific Information
- Profile Requirements : Banner Cover Page completely filled out
- Resume with all facilities, facility size, beds, trauma level, and units listed
- Skills Checklist specific to the discipline (must be less than a year old)
- Active License (verification must be less than 30 days old)
- Certifications (must be active at time of submission)
- Driver's License

- No Nursys Hits - REQUIRED
- Banner has very specific background requirements that must be adhered to and attested.
