YOACAP is seeking a Case Manager for the organization's new community antiviolence program. The program will connect individuals at high-risk of gun violence to resources using trusted community figures : Barbers.
YOACAP will train barbers in West, Southwest and North Philadelphia to identify their clients who are at-risk of gun violence and have conversations with and connect them to YOACAP case manager or other services.
In addition, YOACAP will expand slots in their building trades pre-apprenticeship program for individuals highly at-risk of engaging in gun violence.
The Case Manager will staff the hotline and field inquiries from barbers, community members and youth. The Case Manager will help youth and community members understand what programs and resources are available to them and assist them with navigating the process of obtaining services.
While for some individuals, just being aware of a program or resource maybe sufficient, others may need one-on-one assistance with accessing services and overcoming barriers.
For example, the Case Manager may help individuals complete online application, call agencies or organizations, or gather the necessary paperwork.
If a young person is referred to YOACAP by a barber, family member, or other community members, the Case Manager will work with the youth and supporters to understand their needs and develop a plan to address barriers to receiving services.
The Case Manager may enroll a young person in one of YOACAP's existing programs or provide a warm referral to a partner organization.
The Case Manager will also follow up with youth to ensure they are receiving services and address barriers as they arise.
