Care Manager RN - PMG at Home

Description

As the primary coordinator of the Elder at Home care team, the RN-Care Manager provides across-the-continuum, customized care coordination services and management of transitions of care for the most medically fragile PHP Medicare Advantage, at-risk patients and their families / caregivers, incorporating patient-centered interventions and patient engagement strategies to promote self-management in the home / community setting.

Required Qualifications :

• Oregon Registered Nurse License upon hire.
• 2 years Clinical medical-surgical, acute care or long-term care experience that also includes :
• 1 year in community-based / ambulatory care.
• 1 year in care of the frail elderly.

Preferred Qualifications :

• Bachelor's Degree in Nursing.
• Home health, hospice, palliative care, managed care, or skilled nursing facility background.
• 1 year Care management of diverse patient populations.

About the Team

Providence Physician Enterprise (PE) is a service line within Providence serving patients across seven states with quality, compassionate, coordinated care.
