Our Social Workers are key players in providing support and service to our residents and their families. Our careers offer opportunities to make a positive impact in the lives of seniors every day. Essentials :
- LTC case management
- Discharge planning
- MDS assessments
- Care planning
- Providing resources, and referrals to residents / families / staff members
- Coordinates resident admissions
- Coordinates and leads transfer and discharge team
- Completes assessments of residents as necessary for Care Planning, MDS, RAPS, PPS and other required documents
- Develops and promotes the department through marketing efforts throughout the professional and social community Required Qualifications :
- Bachelor's degree in Social Work or related degree in social sciences; Licensed in the state of Minnesota
- Minimum of one-year of Social Work experience, in a healthcare setting; knowledge of home health care and public health settings;
familiarity with Federal and State regulations and requirements for home health care
- Twenty-one (21) years of age, meet the MVR organization standards, and have a valid state driver's license
- Ability to read, speak and write fluently in English Preferred Qualifications :
