Responsibilities
- Guides the healthcare team toward timely discharge and actively works to eliminate delays in transition
- Engages the patient / family and multiple providers across varied care settings in the discharge planning process to increase the success of transitions to and from care settings
- Manages a specific caseload of patients / family
- Collaborates with the patient / family, and the healthcare and social services teams by promoting access to medical and social systems
- Independently and proactively develop additional processes as needed
- Demonstrates flexibility, ability, and willingness to participate in continuous improvement of processes, workflow, and accountability metrics
- Performs other duties as assigned
Qualifications
- BSW / MSW
- LVN with experience
- 3 years experience in case / care management
- ACM certification within 5 years of hire
