Job Description : We are seeking experienced and compassionate Home Health Nurses to join our team. As a Home Health Nurse, you will be responsible for providing skilled nursing care to patients in their homes under the direction of a physician.

You will assess patients ' health needs, develop and implement care plans, and collaborate with other healthcare professionals to ensure optimal patient outcomes.

Responsibilities : Conduct initial and ongoing assessments of patients ' physical, emotional, and psychosocial needs. Develop individualized care plans based on assessment findings and in consultation with physicians and other members of the healthcare team.

Provide skilled nursing care, including wound care, medication management, and monitoring of vital signs. Educate patients and their families about their health conditions, medications, and self-care techniques.

Coordinate with other healthcare professionals, such as physical therapists, occupational therapists, and social workers, to ensure comprehensive care.

Document patient assessments, interventions, and outcomes accurately and timely in accordance with agency policies and procedures.

Communicate effectively with patients, families, and other members of the healthcare team. Participate in quality improvement activities and contribute to the development of best practices in home health care.

Qualifications : Current license to practice as a Registered Nurse (RN) in the state of Oregon. Minimum of one year of experience as a nurse in a clinical setting;

home health experience preferred. BLS certification required; additional certifications such as ACLS and wound care certification are a plus.

Strong assessment, critical thinking, and communication skills. Ability to work independently and as part of a multidisciplinary team.
