Support new client admissions and perform thorough assessments to ensure tailored care plans. Develop and update individualized care plans to meet evolving client needs. Supervise and mentor field staff, fostering their professional growth and ensuring exceptional care delivery. Serve clients across northern Bronx and Westchester County, with occasional travel to Putnam, Rockland, and Orange Counties. Oversee and validate field staff competencies for new hires and annual reviews, ensuring top-tier performance. Review and maintain accurate, high-quality documentation for all cases. Lead engaging staff training sessions and in-service education to promote continuous learning and excellence. Registered Nurse (RN) license in the state of New York. Bachelor's degree in Nursing (BSN) preferred. Minimum of 2 years of nursing experience. Experience in home healthcare or community health settings. Previous management or leadership experience.
