Director NHTD Waiver Services - Home Health

Compass Healthcare Consulting & Placement is conducting a search for an experienced Director of NHTD, Nursing Home Transition and Diversion Waiver Program for a Home Health Agency located in New York.

Candidate will have the ability to choose which office to work from, offices located in Queens or Brooklyn. The Home Care Agency is very interested in a Director who understands how to develop and grow a NHTD Waiver Program.

Qualified candidates will have a NYS Professional Clinical License, a minimum of 2 years of prior Service Coordination or Case Management experience, will have strong understanding of NHTD Waiver Program.

Prior experience with NHTD Waiver Program, a must! LMSW, LCSW, RN, Occupational Therapist OT, Physical Therapist PT, Speech Language Pathologist SLP or Certified Rehabilitation Counselor CRC with Manager or Director level experience as well as the ability to act as a liaison for patients and families into the NHTD Waiver Program.

The Nursing Home Transition and Diversion (NHTD) Waiver Program uses Medicaid funding to provide support and services to assist individuals with disabilities and seniors toward successful inclusion in the community.

The individual is the primary decision-maker and works in cooperation with providers to develop a Service Plan. This process leads to personal empowerment, increased independence, greater community inclusion, self-reliance, and meaningful productive activities.

Director of NHTD (Nursing Home Transition and Diversion) Waiver Program

Job Summary :

The Director of NHTD will organize and direct the Agency's NHTD Waiver program. He / She functions under the direction of and reports directly to the VP / Administrator.

• 1-5 years of providing case management / service coordination and information, linkages and referrals regarding community-based services for individuals with disabilities and / or seniors;
• Knowledge of Licensed Home Healthcare & NHTD Waiver Program Rules and Regulations, NYS DOH Regulations
• Knowledge of Documentation and Compliance Guidelines for Waiver Program
• Knowledge of and experience in NHTD Waiver program
• Ideal Candidate will have a minimum of 1 year prior Manager / Supervisor level experience providing case management / service coordination and information, linkages and referrals regarding community-based services for individuals with disabilities and / or seniors

Required NYS Professional License :

Current & Active NYS Professional License in one of the Following : LMSW, LCSW, RN, Occupational Therapist OT, Physical Therapist PT, Speech Language Pathologist SLP or Certified Rehabilitation Counselor CRC with NHTD Program experience, required.

Required Education :

• One of the Following Degrees : Masters of Social Work or Doctorate of Social Work, Masters of Psychology or Doctorate of Psychology, Masters of Gerontology with 1+ years of experience providing case management / service coordination and information, linkages and referrals regarding community-based services for individuals with disabilities and / or seniors;
• OR Bachelor's Degree with 3+ years of experience providing case management / service coordination and information, linkages and referrals regarding community-based services for individuals with disabilities and / or seniors;
• OR Associate's Degree with 5+ years of experience providing case management / service coordination and information, linkages and referrals regarding community-based services for individuals with disabilities and / or seniors;
